Equipment Checkout requirements

Before picking up equipment on July 28th or August 11th, please make sure all of your paperwork is complete.

All players must have before checking out equipment

  • All Player Fees must be paid
  • Signed TVYFL Medical release form (available below)

All equipment must be returned at the end of the season.  You will receive a bill for any equipment not returned to your head coach at the end of the season.


Players in 5th-8th grades must have an Impact Concussion baseline before any contact can occur.  Please call the Portland Clinic at 503.293.0161x4036 Or Providence Sports Therapy at 503.574.6595 to schedule your appointment.  This is a service that is provided at no cost to you.
